Are you unhappy with your smile?  You have come to the right place.  Our priority is to deliver quality, comprehensive dental care in a comfortable and convenient setting.  We look forward to welcoming you to our office and finding out how we can help provide you with the smile you've always dreamed of.  

Here is some information that may be helpful to you for your first visit.  

Location - We are located in Lincolnshire Professional Plaza (200 Cleveland St) next to McDonald's.  Enter through the main double doors and our office is to the left.

Insurance - We are providers for Delta Dental of Iowa and Wellmark Blue Cross/Blue Shield.  Regardless of what dental insurance you have, we will submit insurance claims for you.  Deductible and co-payments are expected at time of service.

Payment - We also accept cash, check, Visa and MasterCard.  Financing is available through Care Credit.  Our staff can help estimate what your costs will be.

Registration Forms - During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know Dr. Clove.  To understand what to expect for your first visit, please read through the forms.  You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.
